Demenia Sings Out! (“DSO”) is seeking to reach out to folk who live at home and would like
to attend the singing sessions but who are currently unable to attend due to the
unavailability of transport.
DSO has set aside funds for a Transport Assistance project which will provide a financial
contribution towards the cost of a taxis to attend the Thursday morning singing sessions at
the Compass Church on the Finedon Road in Wellingborough.
The financial contribution will be limited to £20 for a return taxis fare, to be re-imbursed
against the submission of an appropriate taxis receipt.
The Transport Assistance project will run for an initial period of 12 months up to
30 September 2025 when, if found to be operating successfully and subject to available
funding, will be extended.
If you would like to be considered for funding from the DSO Transport Assistance project,
please provide the following information and hand the completed form to one of the DSO
Trustees or send it via email to dementiasingsout@gmail.com. We will let you know when
your application has been approved as soon as possible.
IMPORTANT
DSO is not responsible for making any of the transport arrangements, that remains the
responsibility of the Carer. Please ask the taxis driver for a receipt for your trip and the cost
will be re-imbursed to you in cash up to a limit of £20.00 per return trip i.e., £10.00 each way
